For new qualifying patients, you can submit a request for patient certification using the form provided on this page. In order to be certified as a medicinal cannabis patient, you must have a qualifying condition as outlined under Illinois's Compassionate Use of Medical Cannabis Pilot Program.

Patient Packet
After placing a request for an appointment, new patients will also need to complete and provide us with their patient packet, which is supplied on our THC Patient Packet Page with instructions.

Integrative Primary Care
The Healing Clinic also offers holistic medicine appointments for qualifying patients AND the general public. Unlike registered cannabis patient certifications, IPC appointments do not require a qualifying condition. These services include:
